Pilots licensed by the Federal Aviation Administration who are arrested for driving under the influence (DUI) or driving while intoxicated (DWI) must report any criminal DUI conviction or DMV administrative action to the FAA.
The FAA must be advised of the following actions within 60 days:
- DUI court conviction
- Revocation, suspension, cancellation or restriction of driving privileges
- Administrative per se orders
- 10-day civil revocations
- Express consent revocation/suspension of driving privileges

Reporting a DUI conviction or driver's license suspension to the FAA does not typically result in the pilot losing his or her license. However, failure to report these actions can prompt the Federal Aviation Administration to suspend a pilot's license or refuse to renew it.
Not every FAA license holders falls within these reporting requirements. License holders who do not have to submit a medical certificate, such as aviation mechanics do not have to report actions that stem from a DUI arrest.
To comply with the reporting requirement, send a notification letter (Word file) to the FAA's Security and Investigation Division within 60 calendar days of the effective date of a DUI court conviction or administrative action resulting from a drunk driving arrest to:
